What our forms collect

The Get a Custom Quote form collects your name, work email, optional phone number, practice or company name, role, practice type or specialty, number of providers, states, the payers and services you need, your desired start, optional notes and your consent to be contacted.

To limit spam, we store a one-way hashed version of the network address a form was sent from. We do not store the address itself.

What we ask you not to send

Please do not include patient information, provider documents, NPI numbers, logins or passwords in our forms or by email. When an engagement needs documents, we agree a secure method with you after scope is confirmed.

Why we collect it

We use what you send only to respond to your request: to prepare a quote and follow up with you about that request.

Who can see it

Form submissions are stored in our website database and can be read only by our signed-in account owner. We use service providers to host the website and its database.

We do not sell your data, and we do not share it with third parties for their marketing.

Cookies and tracking

The website does not use analytics or advertising tracking. Our admin area uses browser storage to keep the owner signed in.

How long we keep it

We keep form submissions for as long as needed to respond and to keep a record of our conversation, and delete them on request.

Your choices

You can ask us to correct or delete the details you sent at any time by emailing us.
